Online Forms

Below is a list of forms required at the start of each school year. Please complete the these forms on our KIS Online Forms portal at https://sea.kis.ac.th

Primary School Families will require to complete and sign on the following forms:

Enrollment Agreement Form
KIS Data Protection and Privacy Policy
PYP Student ICT Agreement
Personal Data Protection Act (PDPA) Consent Form
Annual Field Trip Permission Form for One Day Trips ( Early Years 1 – Grade 12 )
Annual Sports Tournament and Activities Permission Form ( Grade 2 – Grade 12 )

Secondary School Families will require to complete and sign on the following forms:

KIS Smart Cards

Parents/guardians who have requested the KIS smart cards on the “Student Services Form” will be issued the cards. Students will be issued a KIS smart card when they visit the school shop to purchase a school uniform. Students will be photographed while trying on uniforms and this photo will be used for the ID card. Please make sure you schedule an appointment for uniform purchase.

Each family is entitled to 2 complimentary smart cards in addition to the student smart card. Additional cards can be issued with a small fee of 150 baht per card (excluding card accessories; lanyard and plastic clear case).

Health Portal

ATK testing requirement after long breaks

KIS no longer require KIS community members to undergo routine ATK testing, however, our most recent ATK protocols include provisions for the school to require ATK testing and submissions of those results through the KIS Health portal after long breaks from school. These periods will be announced in advance.

Please be reminded of our COVID-19 protocols

Those with symptoms must test and report to the KIS Health Portal. No one with symptoms may enter campus without submitting a negative
ATK test
Students and staff who test positive will still require a 5-day quarantine or until the ATK result becomes negative
Parents are asked to continue to be diligent in observing symptoms. If anyone in the household is ill, or symptomatic, students must be kept at home
Face masks are optional both indoors and outdoors
We will continue to share real time tracking of our community through our Health Portal.

School Uniforms

Once your child has been offered a place at KIS, you will be invited to attend the school uniform purchase at KIS school shop. It is important that you bring your child along to have the uniform fitting done. While making the uniform purchase, your child will be photographed in the school polo shirt and this photo will be used for their school ID card (KIS smart card).

Car Stickers

Parents/guardians who have requested car stickers on the “Student Services Form” will be handed the stickers when they visit the school shop to purchase school uniforms. Each family is entitled to 2 complimentary car stickers. Additional stickers can be issued with a small fee of 50 baht.
